Resolved Help With Identifying NuBus Ethernet Card

I recently acquired a NuBus ethernet card to install in my Macintosh IIci. Unfortunately I did not get the drivers with the card. After much searching I have not been able to locate the drivers. Possibly because I do not know much about the card. I've attached pictures in the hope someone can identify the card and, more importantly, point me in the right direction for the drivers.

The only distinctive markings on the board are "EP11T" and "Level One" on one of the chips (see pic). Other than these marks there's nothing distinctive to search on.
