No fans were active until the sim started. Every time the sim is up the MacBook chokes. This has been the case since Xcode 6/7.

Xcode Version 11.0 (11A419c)
Open blank project / no other apps running.

Well it’s simulating an extra operating system with all its own frameworks and such. One which primary focus is ARM not x86. And it tries to allocate enough resources to it to ensure a smooth experience Inside the iOS environment.

I'm using a 2016 MacBook with 8 GB, and while it's not optimal, it's not bad either.
I wonder if your MacBook Pro is due for a cleanup.
Are you running any kind of Antivirus or active firewall?

I use to have that same machine. It’s fans would turn on every time Xcode indexed or I used simulator. The solution that worked for me was to disable turbo boost. It kept the MacBook Pro a little cooler, thus keeping fan speeds down. Here’s what I used. http://tbswitcher.rugarciap.com/
