It should be the standard process of creating an Apple ID (https://appleid.apple.com/) then select "Create your Apple ID" at the top of the page.
Once your account is created, if you do not want to turn on 2FA.. (which in today's world not sure why you wouldn't want this feature on) .. make sure you don't select 2FA and create your security questions.
