I just started ripping DVDs for my ATV. Most work fine but I have had a couple that the audio is out of sync with the video.

I use mactheripper and then Handbrake. I took the same file from mactheripper and used visualhub and the audio was fine.

Am I doing something wrong?

I have had this issue a few times. As a work-around (to avoid re-ripping the DVD) you can go to the audio settings and turn off the digital audio. This will force the ATV to fall back to the stereo/PLII audio stream which typically does not have sync issues.
Obviously you lose 5.1 audio, but at least you'll be able to watch the movie.
