How does Apple send payments for developer account owners?

Do you have an iTunes Connect account (that is, are you a registered iOS Developer)? If so, you should be sure to check out the "Payment and Financial Reports" FAQ as well as all the other FAQs and the iTunes Connect Developer Guide.
How: You give them a tax ID and a bank account number that accepts wire transfers.
When: They have up to 45 days after the close of their fiscal month to deposit a payment into your bank account, but there is often a minimum before they do so, and they are usually a few days faster.
But the actual legal contract (which they may have changed from the above) is one you have to agree to on iTunes Connect. So read it first.
