I had .jpg in the docked Downloads folder. After I dragged the .jpg to another folder a “blank paper” icon remained in the folder. It will not drag to Trash. Also, it’s only visible in Stacks; when I open the folder in Finder it’s not there. CTL-clicking on the icon says "picture.jpg can't be found." When I drag the original .jpg back to the folder, a "-2" is appended to the name so I think it's treating the icon as a file. However, going to the command line and running ls and ls -a doesn't show the file.

Any ideas how to get rid of this thing?

I didn't solve the issue but dragging the Downloads folder out of the Dock (watching it go "poof") and placing it back in the Dock made the icon disappear permanently.
