Mute Switch Functions Backward

My wife has an iPhone 6s running iOS 9.3.4. I noticed while using her phone the other day that her mute switch seems to be vibrating in reverse. When I switch it to silent mode, it shows the bell with a strikethrough, but does not vibrate. When I take it off mute, it shows the bell and vibrates. I tried researching it but could not find any results. Has anyone seen this before? Any ideas on how to fix it?

Vibrate on Silent might be off.

Settings > Sounds > Vibrate on Silent.
