I haven't played with the different in-US, out-of-US, US-account, non-US-account combinations, but there should be no difference in the availability of iTunes Radio on iPhone, iPod Touch 5th Gen, and iPad. I have iTunes Radio on my iPad 3 and on the iPad 2 and iPad minis that I've tested.

Sorry, I can't yet identify the new music app pages at a glance on iOS 7 ;) - expected "more" to be more of a grid view.
Try logging out and in from your iTunes account. If you are in the US, you'll see iTunes radio with the prompt to log in for using. ;)
