iPhone Ringer volume starts out loud and then goes quiet

Hello,
My dad is running an iPhone 8 with iOS 13.5.1. When someone calls him, the ringer volume starts out loud and then a second or so later, the volume drops considerably. This happens regardless of the selected ringtone. My father is hard of hearing, so this is a real problem. Does anyone know why this might be happening and/or how to fix it?
