Why am I getting a prompt to allow or deny Keychain?

I am constantly getting a message which is prompting me to either allow or deny keychain, despite the fact that I never initiated a keychain account. Whenever I attempt to enter a password, it is rejected (I've tried every password I can think of).

Why is this happening and what should I do?
