So, I had Family Sharing setup and was sharing my Apple Fitness+ service with wife and PIL. These were all separate apple ID’s. Now, my wife purchase the apple TV+ subscription, but it doesn’t show up in the family subscription as a shared service? If people are part of a family, do all subscriptions get shared across or is it only the ones that the “organizer” has setup?

If I remember correctly (from Apple Support docs) having the head of family sharing be the purchaser is how Apple noted the plan set up.
As long as the apps allow for family sharing, they should show to all in the family group.
