When online on Safari with my mac air mid 2013 using high sierra 10.13.6 with adguard, I get a message on Youtube saying I am offline and to try again whenever trying to access a video.

No problem using youtube in chrome.

The extension should have a setting to be disabled only for one website, YouTube in this case.

I've had all manner of disastrous performance with YouTube in Safari of late, including the offline message. I use 1Blocker with the optional YouTube script enabled, and had already assumed this was the problem, but frankly I'd rather endure the poor performance than disable blocking on YouTube.
