I think that problem sorted itself out. I had my iMac turned off for a night and when I was in high school. After I came back home and turned it on BT worked much better (my mouse and keyboard were connected much quicker) and lag seems to be gone for now. I need to test it again, this time for longer period of time to be sure. It might also be an antenna. I removed the old one and sticked a new one which looks basically like a thick tin foil with a wire...I placed it exactly where the old one was.

For the lag - You need to check if the antenna is properly installed. I had the same lag until I verified my antenna connections and ensured that the keyboard and the mouse RSSI (Received Signal Strength Indicator - Option + Click on the Bluetooth menu bar icon to see signal strengths is -60 or better - initially it was in the -80 area)

Hey, you installed the dosdude video patches - reinstall and disable auto patching in the options menu of the patcher, add patches later deselecting the video patch. All this could be read on the first page of the correct thread

I tried both cards in two different machines. Unfortunately after a short while at least the (used and cheap) 94331 seems to be faulty. It does not activate the BT part once in a while on boot. The more expensive (used) 94360 works flawlessly. Your link shows exactly the type of adapter and 94360 card I used here, this card will be a used one, too.
You need a three line 50cm cable and a 50cm IPEX 4 antenna cable extension male to female additionally, alternatively you may use a new antenna for BT or use one of the tree WIFI antenna cables for BT. I have tested each case, you get slightly different damping values on your receiving WIFI signal.

What is a single/dual/three/ten line (flat) cable? You need three lines for D+/D-/3,3V. And you need a 50 cm (approx. 20 inch) antenna extension. Please read this not so very long forum thread and look for pictures there. Then you get a better idea of what I was writing about. I just put together the strings.

Did that and it works flawlessly, but I had to keep SIP disabled or partially enabled (csrutil enable --without kext).
Any way to sign kext files so I can fully enable SIP?

Sorry for my question: Why does adapter need the USB connectivity. I see a lot of adapters and some, not all, have the 3 pins, but I can't find information on what their purpose and where do you connect them. Is this something I need to use to make sure I can do unlock with Apple Watch?
Another question: Anyone had experience plugging Broadcom BCM943602CDP card? The bluetooth 4.2 version. Is it compatible with mid 2011 27 iMac and will it give you all the features when installing Catalina?

The third 3.3V line is just necessary to avoid shutdown of BT when you disable WIFI in the system pref pane. With this line the BT part of the card can work while you effectively disabled the power source of the complete card. Somewhere in this thread you find this info...I just copied it from memory.

You need USB connection as USB pins are not connected on the mainboard socket. Therefore, you have to use the same cable that goes to the original BT card. I also cut the trace that connects the 2 power lines (BT and WiFi) on the adapter so I can enable/disable them independently.
You need both BT and WiFi enabled for "unlock with Apple Watch", so you actually don't need that power connection if you plan to keep them enabled at all times (but you still need USB data connections D+ and D-).
I am using Broadcom BCM94360CD card under MacOS High Sierra, and I have full functionality (2011 iMac). Broadcom BCM943602CDP should work as well.

I had an issue with the stock Bluetooth where it would constantly connect/disconnect devices every few seconds so my Apple Keyboard and Magic Mouse wouldn't be usable. Ans yes, you have to sacrifice the old module, at least the cable since exactly this gives you the D+/D- lines and the 3.3V if you want BT do be independent of WIFI. I am asking myself where you are getting the signals from right now??
You cannot have two BT modules active in the system at the same time, there is an Apple software solution to switch back and force, but this is another story. And you can also chose one BT card permanently.

Out of the 2 cards I had one had a faulty Bluetooth. On the one that worked, I also messed up D+/D- first time but then I inverted and all was ok since... The second card which had faulty Bluetooth I sent back for a replacement.
During my installation, I always switched off the original Bluetooth module, and also I took the D+ and D- straight from the Bluetooth cable. I think in one of the previous posts someone replied that you cannot have 2 devices on the same D+/D- 3.3.V line (so perhaps you want to try to do it without the SD card being connected too?!)
I also made a fully reversible solution where I used the connector (took it out) from spare original Bluetooth card to create an extension cable to the new adapter for the BCM94331CD

Currently I do not know how your problem can be solved. From China I got ten cards, three of them had a faulty BT module. But you may face a different problem, since you got your lines from the SD card and probably the old BT module is still active?
You can pull the plug of this module to deactivate it permantely, use the cable to get the signals (same result) or use a software to switch between both modules if both are still connected to an antenna...
I added a new optional step 9 to my guide here. You will find there how to select one particular out of two installed and working modules.
